A 50 year old female came to casualty presented with complaints of shortness of breath, decreased urine output,bilateral pedal edema since 5 days.
HOPI
patient was apparently asymptomatic 5 days back and developed sob decrease urine output pedal edema and complaints of abdominal pain since one week radiating to back
PAST HISTORY
known case of CKD since one year , HTN since 1 year , not a known case of DM and asthma

Tab.OROFER XT OD
Tab.LASIX 40mg OD
Tab.PAN 40mg OD
Tab.NICARDIA 20mg BD
Inj.ERYTHROPOITIN 4000U weekly once
Inj.IRON SUCROSE 100mg weekly once in 100 ml NS
SALT AND WATER RESTRICTION
