A 35 year old female came with the headache,neck pain, generalised weakness since 3 days

Chief complaints: A 35 year old female came with the  -complaints of neck pain  and head ache since 3 days -complaints of genralised weakness, giddiness since 3 days  -complaints of shortness of breath on and off since 3 days  HOPI: Patient is fine since 10 years now she is complaining of neck pain ,headache since 3 days and associated with generalised weakness ,giddiness since 3 days patient is feeling breathless- having breathless episodes and is then waiting up from sleep, subsides in 3 to 5 minutes, SOB on walking grade 2 on and off since 3 days, outside HB 5.0

Case History and Clinical Findings: 55 YEAR FEMALE K/C/O OF DM SINCE 3 YEARS ON OHAS PATIENT CAME WITH C/O GIDDINESS SINCE YESTERDAY MORNING FOLLOWING EXOPOSURE TO HEAT AND WENT TO RMP -SBP WAS 70MMHG AND 10 NS GIVEN -AND SHE DEVELOPED FACIAL PUFFINESS ,PEDAL EDEMA. Drooling of saliva(mild) noticed during presentation At 12:00pm patient GRBS was 25mg/dl @ admission later 25D was given and GRBS increased to 283mg/dl and having hypoglycemic seizures .As per attenders he was unconscious since 10pm(attenders noticed tightening of tongue and snoring) HOPI: Patient was apparently asymptomatic 15 yrs back.Then he got addicted to alcohol since 15 yr
